After you have called and scheduled your appointment with Honet Dermatology & Cosmetic, we would appreciate your organizing and bringing with you any information that may be pertinent to the skin issues you need addressed at the time of your visit.

For All New Patients (Medical & Cosmetic):

All New Medical and Cosmetic Patients need to print, fill out, and sign Forms 1, 3, and 4. Please bring these signed forms to the appointment.

Please read and review Form 2. You may print this one for your own future reference if you wish.

If you are a New Cosmetic Patient, please continue with additional forms that follow.

For All New Cosmetic Patients:

All New Cosmetic Patients also need to print, fill out, and sign Forms 5 and 6. Please bring these signed forms to the cosmetic appointment.
